MBBS reporting documents: 2026–27
The checklist requests NEET and school records, domicile and identity evidence, certificates from the last institution, medical fitness and applicable category, income, migration or gap evidence. Bring originals, two sets of self-attested photocopies and four passport-size photographs.

The bundle includes an undertaking for pending originals, student and parent anti-ragging forms, and a gap affidavit. Admission remains provisional until required documents are submitted and verified.
Some templates contain pre-filled January 2026 dates and a 2024 school-passing year. Confirm the correct entries with the academic office before signing.
The notice dated 11 July 2026 addresses candidates provisionally selected through BOPEE for B.Sc. Allied and Health Care courses. It asked that group to report by 17 July 2026 at 5:00 p.m.
The checklist distinguishes the diploma evidence needed for lateral-entry candidates and asks for admission, identity-card and joining forms.
