Use the correct admission-year route

Health records contain sensitive personal information. Personnel must follow confidentiality, minimum-necessary access, approved retention schedules, secure disposal, correction procedures and incident-reporting rules. Records must never be altered to conceal an error or supplied to an unauthorised person.

The treating clinician remains responsible for clinical documentation and medical decisions. A record technician may identify missing or inconsistent documentation for authorised correction but must not invent, reinterpret or modify clinical facts. Legal requests, police correspondence, insurance disclosure and court records require approved institutional procedures.

Students should understand that privacy obligations continue after duty hours and after employment ends. Curiosity is not a valid reason to access a record. Suspected privacy breaches, identity mismatches, duplicate records or unsafe sharing must be reported through the organisation's authorised process.

Official verification

Sources to check before acting

Rules, dates, fees, seats and recognition can change. Use the current official document for the admission year and keep the evidence supporting each important decision.
